An Asthma Action Plan provides written directions and guidelines to help a person manage his or her asthma. A Plan is developed jointly between the patient and the health care provider and is written by the health care provider. It is specific to each patient. Usually one copy of the Asthma Action Plan is given to the patient or the patient's family, the health care provider keeps one copy, and (for children) one copy is given to the patient's school.
Asthma Action Plans come in many different versions, but all should include:
- List of the patient's asthma triggers
- Recommended doses and frequencies of daily medications
- Symptoms to be aware of and how to adjust medicines in response to those symptoms
- Patient's personal best peak flow, peak flow levels and peak flow zones
- Emergency telephone numbers
Massachusetts Asthma Action Plan
We have included a copy of the Massachusetts Asthma Action Plan, which was adapted from the National Institute of Health's 2001 version. Massachusetts Plans are available in eight languages; we have provided an example of the English and the Spanish version below.

Children's Hospital Boston Asthma Action Plan
Children's Hospital Boston Asthma Action Plan is adapted for children and adolescents from the Massachusetts Asthma Action Plan. Children's Hospital Boston Asthma Action Plans are available in English and the Spanish.
